Medical Claim Process

Outpatient Reimbursements

Required documentation/information

• Duly filled and signed claim form by both claimant and doctor.
• Original receipts with all costs clearly broken down for all services obtained (consultation, laboratory / diagnostic tests and drugs)
• Copy of the prescription from the doctor
• Fully completed bank account details/ Mpesa form

Inpatient reimbursement

Required documentation/information

• Duly filled and signed claim form by both claimant and doctor
• Original receipts for the payment made.
• Discharge summary or medical report
• Itemized bill for services billed
• Fully bank account details/ Mpesa form

Last Expense Reimbursement claims

Required documentation/information

• Copy of burial permit/death certificate
• Copy of beneficiary ID
• Fully completed beneficiary bank details / Mpesa Form

All claims will be considered for settlement if they are forwarded fully documented and received within 45 days from the date of treatment. Claims received after 45 days will be time barred and returned unpaid.
